Case Manager / Housing Specialist (2 Openings)

Job Summary: Responsible for intake, monitoring, and maintaining OASAS NY/NY III clients in scatter-site housing program.

Principal Duties & Responsibilities:

  • Conducts intakes, assessments, quarterly re-assessments, monthly (at minimum) home visits, and develops and implements initial, annual, and quarterly service plans with each client on case load of 25 adults
  • Completes initial, annual, and updated apartment inspections
  • Sets up apartment furnishings and ensures apartments are in move-in condition and ready for occupancy within 10 days of vacancy
  • Coordinates move-in for new clients and relocations for existing clients when necessary
  • Assists clients with establishing utility services and monitors monthly payments to ensure utilities are not disconnected for non-payment
  • Calculates, collects, and tracks monthly rent payments from each client; ensures each client makes appropriate monthly rent payments
  • Resolves all apartment repair/maintenance needs via BAC maintenance staff and advocacy with clients’ superintendents, building managers, and landlords
  • Conducts bi-annual case conferences with collateral providers
  • Provides referrals for various services as necessary
  • Facilitates and co-facilitates tenant meetings and Independent Living Skills groups
  • Compiles and maintains resource directory including landlords, brokers, and service providers; maintains positive working relationships with collateral contacts
  • Supervises interns as necessary
  • Other related duties as assigned

Qualifications Required:

  • Bachelor’s Degree in social work, human services, or related field OR 4+ years’ experience providing housing and/or case management services to homeless population or populations with similar issues
  • Knowledge of issues related to substance use, recovery, and mental health
