JOB TITTLE:                          Patient Navigator

JOB SUMMARY:               Patient Navigator for the Harm Reduction (HRM) Program is responsible for working closely with the Harm Reduction Counselors delivering largely field-based targeted case finding, client engagement, accompaniment, outreach for reengagement, and retention activities to patients enrolled in the Harm Reduction Program, under the direction on the Program Management.  Conducts home visits, phone calls, emails, outreach letters or text messages activates.  Provide critical feedback to other members of the healthcare team.  Conduct HIV Rapid Testing as needed.

QUALIFICATIONS REQUIRED:   Minimum HS Diploma/GED.  Ability to travel frequently for client outreach and home visits.  Must be comfortable traveling to where clients stay (shelters, SRO’s, private apartments, etc.) for outreach and home visits to facilitate patient access to client services.  Basic knowledge of HIV/AIDS prevention, transmission, and treatment and substance abuse.   Bilingual English/and or (Spanish, French, Creole) a plus.  Must be willing to obtain peer certification training recommended by the AIDS Institute.  Basic proficiency with Microsoft Office, HER experience strongly preferred.
