JOB TITTLE:                       Senior Health Educator/Outreach Worker


Responsible for working closely with the Outreach Team delivering largely field-based outreach, retention, and education activities to patients enrolled in all agency programs.  Responsible for improving linkage and promoting access to care.  Compiles and maintains HIV and Medical Health Records, document outreach and educational activities and complete reports of monthly activities.  Data entry into electronic health record (EHR) system.


At least 2 years’ experience conducting outreach, accompaniment, and follow-up activities for participants in a healthcare or CBO setting.  Associate degree or a one year certificate from a college or technical school.  Ability to travel frequently for patient outreach and home visits wherever the patients stay (shelters, SRO’s, private apartments, etc.) for outreach and home visits to facilitate patient access to clinic services.

Experience with HIV Rapid Testing Protocols is preferable.  Bilingual is a plus.
